How to Get Started

Parent calls Hogg Therapy to answer the initial intake questions about their child's needs

OR

Doctor sends the referral and our office calls the family to answer the intake questions (Requested if not already received)

Initial paperwork is emailed to the caregiver. The paperwork must be filled out before we can schedule your appointment. This is to ensure that all the necessary information is received in order to complete and bill the evaluation. 

Once the initial paperwork is sent back to HTP, our intake coordinator will call the caregiver to get the child scheduled.

A reminder text will be sent the day before the evaluation appointment to confirm the appointment. 

Evaluation day, we can't wait to work with you and your child!
