When you arrive for your first visit, you will have the opportunity to talk through your concerns with your therapist and establish a treatment plan catered to your child’s needs!

Give us a call
Our Referral Intake will be able to help you obtain a referral from your pediatrician if you don’t have one already!
Complete New Patient Paperwork
After we receive your child’s referral, our Referral Intake team will send you our New Patient Paperwork. Don’t forget to include your child’s insurance card when you return it!
Schedule an Initial Evaluation
Once we receive your child’s referral, completed new patient paperwork, and insurance information, our Patient Intake team will contact you to schedule your child’s Initial Evaluation.
What to Bring to your child's first appointment:
- Insurance Cards - Please bring both primary and secondary insurances.
- A photo ID (such as a driver's license) to go along with your child's insurance card
- An understanding of your child’s insurance benefits
- A copay for Therapy
Therapy payment plans
At Hogg Therapy Pediatrics, we accept most major insurance plans, including Medicaid, to help make care accessible and affordable for our patients. However, please note that we do not currently accept the following insurance providers:
- Caresource Marketplace
- ChampVA
- Cigna (Note: We accept Cigna only for Speech Therapy; it is not accepted for Occupational or Physical Therapy)
- Wellcare Commercial
- Optum Health
We also accept
limited Aetna commercial plans—please contact us to verify if your specific plan is eligible.
For families who prefer to pay out-of-pocket, we offer a
